REGISTRATION

    The primary registrant is the person completing the form and submitting the single check covering the total costs for all activities chosen by the listed registrants. All other persons listed on the form will be cross-referenced under the name of the primary registrant. A single registration packet, to be picked up at the check-in desk at the conference, will be filed under the primary registrant’s name and will contain name tags and activity verification information for all persons listed on the registration form.

    We are not equipped to accept credit cards, so you will have to print out the REGISTRATION FORM and MAIL it with your CHECK. You may print out the registration form and THEN fill it in with a pen, or you may fill it in by clicking the proper boxes and typing in them, and THEN printing out the form. Notice that the headings on the items at the right are abbreviated, but they are color coded to the descriptions lower down on the page.

A little note from the web site maintainence staff:

    There is a difficulty in making a "form" that will print the same size on everybody's computer. If you have any information that will not FIT on the form, please pencil it in ANYWHERE on the paper, even on the BACK. Ken would like to include the hometown and Club name on the name badges of all the participants, so please include that information for yourself and everyone who is coming with you.
